Has anyone ever rented out their own private narrowboat?

What legal/insurance requirements are necessary?

Been mentioned here a lot. You need insurance and your waterways licence needs to be for hire rather than pleasure. Personally I would have thought it prohibitive unless you are thinking of renting it to someone long term.
